Eligibility, Plan Documents and New Enrollment Information

Upon the completion of one month of service as a regular employee with Stoll Berne, an employee becomes eligible to participate in the Employees’ Profit Sharing Plan. Please review the Summary Plan Document for a description of the Plan.

To enroll in the Plan, you need to complete the following:

1. Summary Plan Receipt Form;
2. Enrollment Form; and
3. Designation of Beneficiary Form.

Enrollment will become effective on the 1st of the next month following the completion and submission of these documents to the Director of Administration. Please hand in your forms at least one week prior to the end of the month to allow enrollment to commence at the beginning of the next month.

Please note that voting rights are not passed through to participants as the 401(k) Plan funds are pooled together. The Trustees reserve the right to exercise or not exercise votes on behalf of the plan.

How to Make Changes to Your 401(k) Investments

To make changes to your plan, you need to complete the Change Form.

Changes will become effective on the 1st of the next quarter following the completion and submission of the Change Form to the Director of Finance & Administration. Please hand in your form at least two weeks prior to the end of the quarter to allow such changes to be made to your plan.

You may cancel your salary deferrals at any time but can only make changes to the amount of the deferral, reallocate your funds, or re-enroll on a quarterly basis.

To change your listed beneficiaries, please complete the Beneficiary Change Form and submit directly to Ascensus via your personal online login. The firm does not manage any changes to your beneficiaries.

The Plan provides you with the option to invest funds deducted from your pay check before taxes (401(k)) or after taxes (Roth 401(k)). The funds in which you can choose to invest include:
